Community Health Improvement Plan
Our community's current CHIP is being carried out through 2025 & 2026 by committees; these are led by community partners and local stakeholders helping the community's leaders take strategic next steps to structure health programs, policies, and services that are informed by what we learned from the 2024 needs assessment.

The Community Health Improvement Plan (CHIP) and the Community Health Needs Assessment (CHNA) is a cyclical 3-year process crafted to give the Pottawatomie County community space to give their say on the biggest needs regarding health and healthcare.
The process informs pivotal decisionmakers and organizations on strategic next steps as they structure future health programs, policies, and services, and provides a structured time scope to take steps forward often.

Food Access & Nutrition
Goal:
Create a food environment that offers equitable access to safe, healthy, and affordable good and encourages consumption of healthy foods.

Mental & Behavioral Health
Goal:
Build a strong, connected network of behavioral health and primary care supports so residents can more easily find help, talk openly about mental health, and feel confident navigating local resources.

Neighborhood & Built Environment
Goal:
Enhance neighborhoods and built environment to promote physical activity, active transportation, social connection, public transportation, and access to green spaces.
