Turning Community Voices into Action: Building Our Community Health Improvement Plan

Over the past several months, we’ve been listening, and now we’re acting. Thanks to the voices of community members who participated in last year’s Community Health Needs Assessment, we’re taking purposeful steps to respond to what we have learned.
We’ve held three key meetings so far to shape our Community Health Improvement Plan (CHIP):
A Stakeholder Engagement Meeting brought diverse perspectives to the table.
An Accountability Meeting helped us draft goal statements and identify shared priority activities.
Up next: A Strategy and Implementation Meeting to turn ideas into action.
We’re proud to be working with over 40 partners, including local businesses, church, tribal healthcare, education, elected, municipal government, and nonprofit leaders, who are committed to advancing health and well-being in our community.
